Custom facial implants are designed to match a patient’s unique facial architecture and are placed to augment your bone structure in areas where it is lacking. These boutique implants can provide added projection and contour to add sharp definition. They are made from advanced biocompatible materials for a safe, long-standing result. Unlike standard ‘off-the-shelf’ implants, they are designed using advanced, state-of-the-art imaging technology that maps a three-dimensional virtual model of your face. Implants are fabricated from safe, medical-grade materials such as silicone, porous polyethylene or PEEK (polymethyl-ethyl ketone). Implants are frequently fixed to the underlying facial bones with a titanium screw.

Custom-designed facial implants offer unparalleled ability to address imperfections in the facial skeleton that contribute to loss of facial volume. During your consultation for facial implants Surrey plastic surgeon, Dr. Imran Ratanshi, will evaluate your specific concerns, assess your facial anatomy, and tailor a treatment plan that works best for you. If having smaller than normal facial bones is thought to be contributing to your concerns, a specialized CT scan can be ordered by Dr. Ratanshi to map your facial bones.
This is done at a private imaging facility (fees are paid by the patient directly to the center. A facial bones CT starts at $700.00 CAD + GST). A digital copy of the imaging is then provided to Dr. Ratanshi to formally review your skeletal anatomy and formalize a strategy to address your concerns. Once it is confirmed that a custom facial implant is the best option for you, the design process can take place and your implant can be ordered.
Implants are then designed by Dr. Ratanshi after considering your specific goals and weighing them against established relationships to restore facial balance. These can vary by gender and by ethnicity. The final implant design will be reviewed with you before ordering for final approval. Once approved by you, your surgical date can be arranged. The procedure is performed under general anesthesia to ensure your comfort and safety.
You will be put to sleep by a board-certified Anesthesiologist. Concealed incisions are designed based on the location of the implant. These incisions enable exposure and development of an implant pocket and ‘press fit’ placement of your implant to your facial bones. A titanium screw may be placed to help secure it in place. You may require a small drain to be removed within a few days of surgery. Multiple layers of dissolving stitches are used to close your incisions. Sutures that require removal may be placed if an eyelid incision is required. Surgical tapes, dressings or bandages are applied to help with swelling.
Scars are intended to be truly ‘invisible’. Depending on what area you wish to enhance, scars are hidden using established techniques in craniofacial surgery. These include the inner eyelid and inside the mouth. In some cases, a scar hidden below the undersurface of chin is used or lateral eyelid.

Your results will be immediately noticeable. If incisions are concealed within your upper or lower gumline (inside the mouth), a lukewarm, blenderized diet is ideal for 1-2 weeks after surgery. You should rinse your mouth after each meal or non-water drink to reduce debris accumulating along your incisions. A mouth rinse every 4-6 hours is recommended. You may brush your teeth after 24 hours, but avoid brushing directly along your incision to avoid irritation and interference with healing.
Expect some swelling bruising and discomfort which can be managed with prescribed medications. You'll need to limit strenuous activity for several weeks, but light walking is encouraged to promote circulation. Regular follow-ups with your Dr. Ratanshi to monitor your healing process will be organized. While initial results are visible immediately, the final outcome can be expected after swelling subsides, usually within 6 to 12 months. Follow-up appointments are scheduled the next day after surgery, 1 week, 2 weeks, 6 weeks and for long-term follow-up.
The cost of your custom facial implant and place depends on the size, location, number of implants, and time required to place them. Prior to developing the implant, the implant fee and cost of design must be paid in full.
Revision surgery for an implant placed by Dr. Ratanshi is subject to its own independent surgical fees, depending on the time required to address the concern. Custom implant surgery to address a cosmetic concern is NOT a benefit of the British Columbia Medical Services Plan.
Our office accepts a variety of payments including major credit cards, bank drafts, or debit cards. For any credit card transactions, a 3% service fee would apply. For any credit card refunds, this 3% fee is also applicable. Dr. Ratanshi has partnered with Beautifi and iFinance Canada to provide financing options for patients looking to pursue surgery.
If you have concerns, it is always better to see the surgeon who initially performed your surgery. Your surgeon is most familiar with your medical history, initial goals and expectations, and execution of your surgery. He or she is best suited to guide you in your early recovery.
If not in the early phase (i.e. more than 12-18 months have passed since your surgery), Dr. Ratanshi can assess you. Your surgical plan may involve re-imaging and possible removal of your implants. If you require new implants, a new planning session is required. Dr. Ratanshi may not be able to use your old imaging as modifications may have been done, or imaging may not be to standard to perform advanced virtual surgical planning.
If you have been injured in a motor vehicle collision and your file has not been paid out, you may be eligible for coverage of your implant surgery or scar revision under ICBC.
