With age, the upper eyelids can develop a more tired, weathered appearance from the accumulation of extra skin. This can be exaggerated if the outer eyebrow descends below the bony rim above your eye. Weakness in the supporting structures of the eyelid may cause the fat that cushions the eye to bulge outward. There is an important gland in the upper, outer portion of your orbit (the bony space containing the eye) called your lacrimal gland. This contributes to tear film formation. In some cases, the ligaments supporting the lacrimal gland can lengthen and the lacrimal gland can drop, creating fullness in your upper, outer eyelid. Management of your gland can be addressed with your upper eyelid surgery if needed.
Upper blepharoplasty may be combined with brow lift surgery to address eyebrow descent (which can contribute to bunching of upper eyelid skin), or lower blepharoplasty if there are lower eyelid concerns.

Upper blepharoplasty takes approximately 30 to 60 minutes to perform. Prior to your surgery, Dr. Ratanshi will make markings to help plan the degree of lift required. Eyelid Lift Surgery is a procedure performed that may be performed under local anesthesia (i.e. local freezing) alone for simpler cases, or general anesthesia if more involved or if combined with other procedures.
An incision is designed by Dr. Ratanshi to camouflage the final scar in your upper eyelid crease. The location of the scar and degree of skin removed can be customized depending on your ethnicity and goals.
If General Anesthesia is required, you will be put to sleep by a board-certified Anesthesiologist. Local freezing medication is also used to help maintain your comfort. Your incisions are closed in multiple layers to offload tension and heal with a fine-line scar. Often a stitch is placed to support your incision that requires removal at one week. In some cases, an ultrafine dissolving suture may be used.
Following upper eyelid surgery, patients can expect some swelling, bruising and mild discomfort which can be managed with pain medication and cold compresses. Most individuals are able to resume their normal activities within a week, although strenuous exercise should be avoided for several weeks.
As swelling subsides and the healing process progresses, you will begin to notice the full effects of your upper blepharoplasty. Over time, incision lines will fade leaving you with a natural-looking result that enhances your overall appearance.

Upper eyelid surgery is a cosmetic surgery to improve the appearance of the upper eyelid. It is not a benefit of the British Columbia Medical Services Plan for most individuals. There are some patients who may experience significant descent of their eyelids to the point it is obstructing their vision. Dr. Ratanshi accepts referrals from primary care providers for this provided the degree patient’s visual obstruction and candidacy for MSP-covered upper Eyelid Surgery has been confirmed with visual field testing by an Ophthalmologist certified by the Royal College of Physicians and Surgeons of Canada. This consultation must be included with your referral from your Family Physician.
If management of the Eyebrow is required, such as with an Upper Eyelid Blepharoplasty or Lower Lid Blepharoplasty, additional fees would apply. Dr. Ratanshi will provide you with a detailed quotation during your consultation process.
